Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing to know about pairing Evodia with fosaprepitant (Emend): the interaction works mostly in the opposite direction from what you might expect. Fosaprepitant is converted in your body to aprepitant, which gently blocks a liver enzyme called CYP3A4. Evodia's active compounds (like evodiamine) are cleared by that same enzyme. So by slowing it down, fosaprepitant could let Evodia build up higher and linger longer in your system.

This is theoretical and comes only from lab and animal studies, so it isn't a proven problem in people. Still, since Evodia isn't well studied for safety, I'd just run it by your pharmacist or oncology team before combining them, especially during chemo when you're already on a lot.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the basic idea with Emend (fosaprepitant), the IV medicine often given to prevent nausea and vomiting from chemotherapy. Your body relies partly on a liver enzyme called CYP3A4 to clear this drug. In test-tube studies, evodia seems to slow that enzyme down. If that also happened in people, fosaprepitant could linger a bit longer and feel stronger, which might mean more of its usual effects like tiredness, fatigue, hiccups, or constipation.

I want to be honest, though: this is only theoretical, based on lab research, and it has never actually been shown in humans. So I'm not alarmed here. Still, since fosaprepitant is usually given during cancer care, it's worth telling your oncology team or pharmacist before adding evodia so they can keep an eye on things.

Here's the thing to keep in mind with this pairing: the concern runs the opposite way from what you might expect. The interaction isn't about Evodia changing how Fosaprepitant (Emend) works in your body. It's the reverse. Fosaprepitant gets converted to aprepitant, which can speed up a liver enzyme called CYP3A4. Since that same enzyme appears to help break down Evodia's active compounds, taking them together could lower Evodia's levels and make it less effective.

Honestly, this is mostly a theoretical worry based only on lab and animal studies, so I wouldn't lose sleep over it. Your antinausea treatment isn't the one at risk here. Still, if you're using both, just mention it to your pharmacist or doctor so they can take a quick look.
