Ivacaftor, Lumacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

The ivacaftor part of Orkambi leans on a liver enzyme called CYP3A4 to be cleared from your body. In lab dishes, evodia seems to slow that same enzyme down. If that held true in people, ivacaftor could build up a bit, which might bring on side effects like headache, nausea, stomach upset, dizziness, or rash. It's worth knowing that lumacaftor actually speeds CYP3A4 up, so this is a bit of a mixed bag.

Here's the reassuring part: this is only a test-tube finding and has never been shown in humans, so it's theoretical. Still, since Orkambi is an important medicine, I'd check with your CF team or pharmacist before adding evodia rather than starting it on your own.

Theoretically, taking CYP3A4 inducers might decrease the levels and clinical effects of evodia.
Animal research shows that concomitant administration of dexamethasone, a known CYP3A4 inducer, with the alkaloid constituents of evodia significantly reduces the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the key thing to know with Orkambi (lumacaftor and ivacaftor): the lumacaftor part actually speeds up a liver enzyme called CYP3A4. The interaction we have on file flows in that direction. Evodia's active compounds seem to be broken down by that same enzyme, so taking them together could clear the evodia out of your system faster, leaving less of it around to do anything. In plain terms, the worry here is that your Orkambi might make an evodia supplement weaker, not that it harms your Orkambi.

This is very early, theory-and-animal level evidence, so I wouldn't lose sleep over it. There's no clear sign it changes how well your Orkambi controls your cystic fibrosis. Still, since evodia isn't well studied in people, please run it by your pharmacist or CF team before adding it.

About Lumacaftor and Ivacaftor

What Is Lumacaftor and Ivacaftor Used For?

Brand name: Orkambi

Orkambi is approved to treat cystic fibrosis (CF) in patients age 1 year and older who have two copies of the F508del mutation in the CFTR gene — one on each chromosome. This is called being homozygous for F508del. If a patient's genetic type is unknown, a lab test approved by the FDA should confirm the mutation before starting treatment.

Orkambi has not been proven safe or effective for people with CF who have a different mutation pattern. It is only for the F508del/F508del genotype.
