Why this applies to Macitentan, Tadalafil
Macitentan, Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi is a combo pill that pairs macitentan with tadalafil, and both of those ingredients are cleared by a liver enzyme called CYP3A4. In lab dish studies, evodia seems to slow that same enzyme down. If that were to happen in your body, these two medicines could build up a bit higher than usual, which might mean stronger blood-pressure-lowering effects and side effects like headache, flushing, dizziness, low blood pressure, nasal congestion, or swelling in the legs.
Here's the reassuring part: this is only a theoretical concern based on test-tube work, and it has not been shown to happen in real people. Neither drug is a prodrug, so the direction would be more effect, not less. Still, since this is a serious heart and lung medication, it's worth a quick chat with your pharmacist or doctor before adding evodia.
