Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Here's the concern with pairing Evodia and pomalidomide (Pomalyst). Your liver uses an enzyme called CYP1A2 to help break down pomalidomide, and Evodia (through a compound called rutaecarpine) appears to speed that enzyme up. If pomalidomide gets cleared from your body faster, its blood levels can drop, which could mean it works less well against your myeloma or Kaposi's sarcoma. With a cancer medicine, you really don't want it quietly underperforming.

I'll be honest about the evidence: this is based on lab and animal studies, not human trials, so we can't say exactly how big the effect would be in a real person. Still, because pomalidomide is such an important treatment, I wouldn't risk it. Please talk with your oncology team or pharmacist before using any Evodia product, and don't change your medication on your own.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Pomalidomide (Pomalyst). Your body relies partly on a liver enzyme called CYP3A4 to clear this medication out of your system. In lab studies (test tubes, not people yet), evodia appears to slow that enzyme down. If that same thing happened in your body, pomalidomide could clear more slowly and build up, which may mean stronger effects and more side effects like low blood counts, fatigue, infections, or bleeding and bruising.

Now the honest part: this is only theoretical and based on lab work, so we don't actually know if it matters in real patients. But since pomalidomide is a serious cancer drug, I'd keep it safe and run any evodia use by your oncologist or pharmacist first before mixing the two.

Human evidence for these uses is very limited, and it is mostly studied in laboratory and animal research. About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
