Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the thing to know about pairing Evodia with Pralsetinib (Gavreto): this isn't about one changing how the other is broken down. Instead, both can nudge your heart's electrical timing in the same direction. Pralsetinib can lengthen what's called the QT interval, and in lab and animal studies Evodia did the same. Stacked together, the worry is an additive effect that could raise the risk of an irregular, fast heartbeat (including a dangerous rhythm called Torsade de pointes).

That said, the evidence for Evodia here is weak, coming from test tube and animal research, not people, so this is more theoretical than proven. Still, with a cancer drug like this I'd play it safe. Please check with your oncology team or pharmacist before adding Evodia.

Pralsetinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the simple version. Your body relies on a liver enzyme called CYP3A4 to break down pralsetinib (Gavreto) and clear it out. In lab studies (test tubes only), evodia appears to slow that same enzyme down. If that were to happen in people, your body might clear pralsetinib more slowly, letting it build up. That could mean stronger side effects, things like high blood pressure, fatigue, swelling, mouth sores, low blood counts, or liver changes.

The important caveat: this is only theoretical and based on lab research, not anything seen in actual patients. Still, because pralsetinib is a serious cancer medicine, I'd play it safe. Please check with your oncology team or pharmacist before adding evodia.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
