Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing to know about pairing Evodia with ritonavir (Norvir). Ritonavir is a powerful blocker of a liver enzyme called CYP3A4, and that same enzyme helps clear Evodia's active compounds (like evodiamine and rutaecarpine) out of your body. So when you take both, ritonavir can slow that breakdown and let the Evodia compounds build up higher and stay around longer. In other words, the worry runs from ritonavir toward the supplement, not the other way around.

This is based only on lab and animal studies, so it's theoretical, and we don't really know what extra Evodia in your system would do in a person. Since you're managing HIV, please don't add Evodia without first running it past your pharmacist or doctor.

Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Norvir (ritonavir). Your liver uses an enzyme called CYP3A4 to help break this medicine down. Lab studies suggest that evodia may slow that same enzyme. In theory, if ritonavir is cleared more slowly, levels could creep up, which might mean more of its common side effects like nausea, diarrhea, an upset stomach, or a strange taste in your mouth.

I want to be honest with you though: this is only seen in test tube research, not in real people, so it's mostly a theoretical concern. Ritonavir itself is also a very strong CYP3A4 blocker, so its own effect likely overshadows anything evodia would add. Still, since HIV therapy is too important to gamble with, please run evodia past your pharmacist or HIV provider before trying it.

What Is Ritonavir Used For?

Brand name: Norvir

Ritonavir (brand name Norvir) is used together with other HIV medicines to treat HIV-1 infection in adults and children older than one month of age. It is always taken as part of a combination regimen — never on its own.

Norvir oral powder is specifically indicated for pediatric patients with HIV-1 infection. Norvir tablets and oral solution are indicated for both adults and pediatric patients with HIV-1 infection. Ritonavir tablets (generic) are also indicated for the treatment of HIV-1 infection in combination with other antiretroviral agents.
