Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing to keep straight with this pair: it's not your hepatitis C medicine that's expected to change, it's the evodia. Simeprevir (Olysio) can mildly block an enzyme called CYP3A4, and evodia's active compounds (like evodiamine and rutaecarpine) appear to be broken down by that same enzyme. So if you slow that breakdown, the evodia compounds may build up in your body and hang around longer, which could mean stronger or more unpredictable effects from the herb.

I'll be honest with you, this is theoretical. It comes from animal and lab studies, not from people, so we can't say how big a deal it really is. Since you're treating a serious liver infection, I'd loop in your pharmacist or doctor before adding evodia rather than guessing.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Olysio (simeprevir). Your liver uses an enzyme called CYP3A4 to clear this medicine out of your body. In lab dish studies, evodia seems to slow that same enzyme down. If that happened in real life, simeprevir could build up a bit higher than expected, which might mean more of its usual side effects like sun sensitivity (a rash or sunburn-type reaction), nausea, or itching.

The important part: this is only theoretical, seen in test tubes and not in actual people, so we don't know if it truly matters. Still, since simeprevir is a serious hepatitis C drug, I'd play it safe. Please check with your pharmacist or doctor before adding evodia rather than guessing.

What Is Simeprevir Used For?

Brand name: Olysio

  • Simeprevir is used along with ribavirin (Copegus, Rebetol) and peginterferon alfa (Pegasys) to treat chronic hepatitis C (an ongoing viral infection that damages the liver). Simeprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Simeprevir may not prevent the spread of hepatitis C to other people.
