Here is the connection. Iloprost (Ventavis) isn't just opening up the blood vessels in your lungs, it also has a mild blood-thinning effect because it makes platelets less sticky. Fasting has been linked, mostly through scattered patient reports, to a higher chance of bleeding when combined with drugs that thin the blood or affect platelets. So in theory, pairing the two could nudge your bleeding risk up a little.

Honestly though, the evidence here is thin (mostly anecdotal, and the clearest examples involve warfarin, not iloprost). With iloprost you might already notice easy bruising, nosebleeds, or jaw pain, so just keep an eye out for anything that bleeds longer than usual. Before starting any fasting routine, run it by your doctor or pharmacist first, especially since PAH itself needs careful management.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
