Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Sweet Clover interacts with.

Theoretically, fresh sweet clover might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Fresh sweet clover contains coumarinic acids, which are converted to free coumarins during drying. Dicoumarol, which has anticoagulant activity, also has been found in some sweet clover preparations and can be formed if fresh sweet clover is allowed to spoil.

Here's the thing to know about Iloprost (Ventavis): besides relaxing the blood vessels in your lungs, it also makes your platelets a little less sticky, so it has a mild blood-thinning side to it. Sweet clover can add to that. If the plant has spoiled or fermented, its coumarin can turn into dicoumarol, a compound that genuinely thins the blood. Stack that on top of Iloprost, and you could tip the scales toward easier bruising, nosebleeds, or bleeding that's slow to stop.

I'll be straight with you though: this is a theoretical concern based on how these compounds behave, not strong human studies. Iloprost is not a prodrug, so nothing here changes how it's activated. Still, with a serious condition like PAH, please check with your pharmacist or doctor before adding sweet clover.
