Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.

Here's the gist: esmolol (Brevibloc) is a fast-acting beta blocker given through an IV, usually in a hospital or monitored setting to bring down a racing heart rate or high blood pressure quickly. Fenugreek, when taken in concentrated supplement doses, may have its own mild blood pressure lowering effect. Stacked together, the worry is simply an additive drop, meaning your blood pressure or heart rate could dip a bit lower than expected.

I'll be honest, this is rated minor and rests mostly on animal data with another beta blocker, so it's far from proven. Since esmolol is given by clinicians who watch your vitals closely, any change would be caught quickly. A sprinkle of fenugreek in cooking is unlikely to matter. Just let your care team know about any supplements you take.
