Why this applies to Lepirudin

Lepirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Fenugreek interacts with.

Theoretically, fenugreek might have additive effects when used with anticoagulant or antiplatelet drugs.
Some of the constituents in fenugreek have antiplatelet effects in animal and in vitro research. However, common fenugreek products might not contain sufficient concentrations of these constituents for clinical effects. A clinical study in patients with coronary artery disease or diabetes shows that taking fenugreek seed powder 2.5 grams twice daily for 3 months does not affect platelet aggregation, fibrinolytic activity, or fibrinogen levels .

Lepirudin is a powerful injectable blood thinner, given in the hospital to prevent dangerous clots (often in people who can't take heparin). The concern here is simple: fenugreek may have a mild blood-thinning quality of its own, so stacking it on top of Lepirudin could, in theory, add up and raise your bleeding risk. That might look like easy bruising, nosebleeds, bleeding gums, or bleeding that's harder to stop.

The good news is this is mostly theoretical. A clinical study found that fenugreek seed powder did not actually change how platelets clot. Food amounts in cooking are very unlikely to matter. Still, since Lepirudin is closely monitored, please tell your care team before using concentrated fenugreek supplements so they can keep an eye on you.
