Mecamylamine falls under Antihypertensive Drugs — the group Fenugreek interacts with.

Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.

Here's the simple version: mecamylamine (Inversine) is a blood pressure medicine, and fenugreek may have a mild blood pressure lowering effect of its own. When you stack the two, those effects can add up, so your pressure could dip a little lower than expected. With a drug like mecamylamine, that might show up as feeling lightheaded, dizzy when you stand up, weak, or even faint.

I want to keep this in perspective. The evidence here is on the weaker side and mostly from animal studies, so this is more of a 'keep an eye on it' than a red alert. Fenugreek as a kitchen spice in food is unlikely to matter much. Concentrated supplement doses are where it's worth caution. If you take both, check your blood pressure regularly and chat with your pharmacist or doctor before changing anything.
