Fermented Milk and Fosfomycin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Fosfomycin

Fosfomycin falls under ANTIBIOTIC DRUGS — the group Fermented Milk interacts with.

Theoretically, antibiotic drugs might decrease the effectiveness of fermented milk.
Some fermented milk preparations contain live and active organisms, including lactobacilli and bifidobacteria strains. Simultaneously taking antibiotics might kill a significant number of these organisms. Tell patients to separate administration of antibiotics and fermented milk preparations by at least 2 hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing fermented milk (like kefir or live-culture yogurt) with fosfomycin: the worry runs in one direction. Fosfomycin is an antibiotic, so it doesn't care whether bacteria are causing your infection or are the friendly probiotic kind in your drink. It can knock out some of those live cultures, making the fermented milk less effective at doing its gut-friendly job. This won't hurt how well fosfomycin clears your urinary tract infection.

This is a theoretical concern based on how antibiotics work, not something proven to cause real problems. A simple fix is to space them out by about 2 hours. Since fosfomycin is often just a single dose, that's easy to manage. Check with your pharmacist if you're unsure.

From our pharmacist’s overview on the Fosfomycin page

“Fosfomycin (Monurol) is often given as a single sachet to knock out a bladder infection, so you get one chance to absorb the full dose, and that is exactly where this list deserves real care. The clearest hazard is activated charcoal, which binds medicines in the gut and can cut how much fosfomycin your body actually takes in.”

About Fosfomycin

What Is Fosfomycin Used For?

Brand name: Monurol

Fosfomycin is an antibiotic used to treat urinary tract infections (UTIs) caused by specific bacteria. The right form depends on how serious the infection is.

Oral forms (Fosfomycin Tromethamine and Monurol) are approved only for uncomplicated UTIs — also called acute cystitis — in women 18 and older, caused by E. coli or Enterococcus faecalis. They are not for kidney infections or abscesses around the kidney. Contepo (intravenous) is approved for adults 18 and older with complicated UTIs, including acute pyelonephritis (kidney infection), caused by susceptible E. coli or Klebsiella pneumoniae.
