Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thinking behind this one. Tocilizumab (Actemra) calms down an overactive immune system to ease conditions like rheumatoid arthritis and giant cell arteritis. Fermented milk products, like yogurt or kefir, contain live probiotic bacteria that some research suggests can gently nudge the immune system to be more active. On paper, that's the opposite of what tocilizumab is trying to do, so the concern is that fermented milk could slightly work against the medication.

I want to be honest about how soft this is. This interaction is theoretical, based on how things work in the lab rather than real patients on this drug. There's no strong evidence that everyday yogurt will undo your treatment. Tocilizumab also doesn't depend on your liver to activate it, so this isn't a metabolism problem. Still, if you have a weakened immune system, check with your doctor or pharmacist before using high-dose probiotic supplements.
