Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Risankizumab-Rzaa
Risankizumab-Rzaa falls under IMMUNOSUPPRESSANTS — the group Fermented Wheat Germ Extract interacts with.
Animal research suggests that fermented wheat germ extract can stimulate immune function.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking behind this one. Skyrizi (risankizumab) works by calming down a specific part of your overactive immune system to ease psoriasis, psoriatic arthritis, or Crohn's. Fermented wheat germ extract, on the other hand, has been shown in animal studies to do the opposite, nudging the immune system to be more active. So in theory, the supplement could work against what Skyrizi is trying to do, possibly making it less effective.
If that happened, you might notice your symptoms creeping back, like return of red, scaly skin patches, joint pain and swelling, or gut flare-ups. I want to be honest though: this is a theoretical concern based only on lab and animal data, not real patient cases. Still, since this supplement is often used during cancer care, please loop in your oncologist or pharmacist before mixing the two.

From our pharmacist’s overview on the Risankizumab-Rzaa page
“Echinacea captures the concern on this page better than any other entry. Risankizumab-rzaa (Skyrizi) treats plaque psoriasis, psoriatic arthritis, and Crohn's disease by quieting one arm of the immune system, and echinacea, like nearly all 53 ingredients listed here, is taken specifically to stir the immune system up.”

About Fermented Wheat Germ Extract
What is Fermented Wheat Germ Extract?
- Cancer supportive care (as a complementary product)
- Immune support
- General wellness during cancer treatment
- Antioxidant support
- Fatigue during illness
Fermented wheat germ extract is a supplement made by fermenting wheat germ with yeast, and it is most often marketed for people going through cancer treatment. While some early research and a few small studies are interesting, the overall evidence is limited and far from conclusive, so it should never replace standard medical care. Talk with your oncologist or pharmacist before using it, since it is a supplement and not an approved treatment.
