Fermented Wheat Germ Extract and Trimipramine

Here is the interaction we found between these two, with the evidence behind it.

SEROTONERGIC DRUGS

Why this applies to Trimipramine

Trimipramine falls under SEROTONERGIC DRUGS — the group Fermented Wheat Germ Extract interacts with.

Theoretically, concomitant use of fermented wheat germ extract with serotonergic medications may increase the risk of serotonergic adverse effects.
Animal research shows that fermented wheat germ can increase levels of serotonin in the brain.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: trimipramine (Surmontil) is a tricyclic antidepressant that already raises serotonin in your brain to help with depression. The concern with fermented wheat germ extract is that animal studies hint it might bump up brain serotonin too. Stack the two together and, in theory, you could push serotonin a little too high.

I want to be honest about the evidence here, though. This is based only on lab and animal research, not real people, so it's more of a theoretical caution than a proven problem. If serotonin did climb too high, you might notice agitation, a racing heart, sweating, shakiness, or confusion. I wouldn't lose sleep over it, but please run it past your oncologist or pharmacist before combining them, and don't change your trimipramine on your own.

“If you take trimipramine (Surmontil), a tricyclic antidepressant, read this list carefully. Nine of the 139 supplements and herbs on file are rated major, and most touch the heart: several, grapefruit among them, can stretch the QT interval, the heart's electrical reset time, and raise the risk of a dangerous rhythm.”

About Fermented Wheat Germ Extract

What is Fermented Wheat Germ Extract?

  • Cancer supportive care (as a complementary product)
  • Immune support
  • General wellness during cancer treatment
  • Antioxidant support
  • Fatigue during illness

Fermented wheat germ extract is a supplement made by fermenting wheat germ with yeast, and it is most often marketed for people going through cancer treatment. While some early research and a few small studies are interesting, the overall evidence is limited and far from conclusive, so it should never replace standard medical care. Talk with your oncologist or pharmacist before using it, since it is a supplement and not an approved treatment.

About Trimipramine

What Is Trimipramine Used For?

Brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp

Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.

In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
