Here's the gist: nortriptyline gently raises serotonin in your brain to help lift your mood. There's some early animal research hinting that fermented wheat germ extract may also nudge serotonin levels up. So in theory, pairing the two could push serotonin a bit too high and add to serotonergic side effects.

If that happened, you might notice things like agitation, a racing heart, sweating, shakiness, or feeling unusually restless. I want to be honest, though: this concern comes from lab and animal studies only, not from people, so it's more of a caution than a proven problem. Nortriptyline is also a drug we like to keep steady, so it's worth a quick chat with your pharmacist or doctor before combining them, especially during cancer care.
