Vyvgart (efgartigimod) works by lowering certain antibodies in your blood, which calms the immune attack seen in conditions like myasthenia gravis. The concern here is the opposite direction: fermented wheat germ extract is thought to stimulate immune activity. In theory, nudging the immune system in the active direction could work against what Vyvgart is trying to do, possibly making your myasthenia symptoms (muscle weakness, drooping eyelids, trouble swallowing) harder to control.

I want to be honest that this is a theoretical worry based only on animal and lab data, not real-world reports in people on Vyvgart. Still, since this is a serious autoimmune condition, please loop in your neurologist or pharmacist before adding this supplement so they can weigh it for your situation.
