Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Fiorinal (aspirin, butalbital, and caffeine) and chamomile. In lab dishes, chamomile compounds appear to slow down a liver enzyme called CYP3A4, which helps clear part of this medication, especially the butalbital. In theory, if that enzyme runs slower, butalbital could build up a bit and leave you feeling more drowsy, lightheaded, or foggy than usual.
But I want to be honest about the strength of this. This only comes from test-tube work, and it has never actually been reported in real people. A cup or two of chamomile tea is very unlikely to cause trouble. Concentrated capsules are more of a question mark. If you take Fiorinal regularly and want to use chamomile, just run it by your pharmacist or doctor first.

In vitro and animal research shows that German chamomile might inhibit CYP1A2. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP1A2 in patients taking German chamomile.

Here's the piece worth knowing: the caffeine part of Fiorinal is cleared by a liver enzyme called CYP1A2, and lab and animal studies hint that German chamomile might slow that enzyme down a little. In theory, that could let caffeine linger longer in your body, so you might notice more jitteriness, a faster heartbeat, or trouble falling asleep. The aspirin and butalbital pieces aren't really affected through this pathway.
I want to be honest with you though: this has only shown up in test tube and animal research, never in actual people, and it's rated as a minor concern. A cup or two of chamomile tea is very unlikely to cause trouble. If you use concentrated chamomile supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.
