Theoretically, German chamomile might inhibit CYP3A4 and increase levels of drugs metabolized by these enzymes.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.

This combo pill pairs aspirin and caffeine with orphenadrine, a muscle relaxer. The piece that matters here is orphenadrine, since it's partly broken down by an enzyme in your liver called CYP3A4. German chamomile has shown, in lab dish studies only, that it might slow that enzyme down. In theory, that could let orphenadrine build up a bit and bump up its usual side effects, things like drowsiness, dry mouth, blurry vision, dizziness, or a faster heartbeat.

Here's the reassuring part: this has never actually been reported in real people, and the proof is very weak. An occasional cup of chamomile tea is unlikely to cause trouble. If you take concentrated chamomile supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

Here's the simple version. The caffeine in your combo pill (the one with aspirin, caffeine, and orphenadrine) is cleared by a liver enzyme called CYP1A2. Lab and animal studies hint that German chamomile might slow that enzyme down a little. In theory, that could let caffeine hang around longer, so you might feel jittery, have trouble sleeping, notice a racing heart, or feel a bit restless.

The honest part: this has never actually been reported in people, the evidence is weak, and it's rated as a minor interaction. A normal cup of chamomile tea is very unlikely to cause trouble. Still, if you take concentrated chamomile supplements daily, it's worth a quick chat with your pharmacist or doctor.
