Dorzolamide, Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group German Chamomile interacts with.

Theoretically, German chamomile might inhibit CYP2D6 and increase levels of drugs metabolized by these enzymes.
In vitro evidence shows that German chamomile might inhibit CYP2D6. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP2D6 in patients taking German chamomile.

Here's the honest picture for Cosopt (dorzolamide and timolol eye drops). German chamomile has shown, only in lab dish studies, that it might slow down an enzyme called CYP2D6. The timolol part of your drops is handled in part by this enzyme. In theory, blocking it could let a little more timolol build up in your body.

What might that look like in real life? Since timolol is a beta blocker, slightly higher levels could nudge your heart rate down or lower your blood pressure, and in sensitive people affect breathing. That said, this is theoretical and has never actually been reported in people. Eye drops deliver small amounts to begin with. A relaxing cup of chamomile tea is very unlikely to cause trouble, but if you use concentrated chamomile supplements or notice dizziness or a slow pulse, check in with your pharmacist or doctor.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
