Amitriptyline, Perphenazine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Ginkgo interacts with.
Ginkgo seeds contain ginkgotoxin. Large amounts of ginkgotoxin can cause neurotoxicity and seizure. Ginkgotoxin is present in much larger amounts in ginkgo seeds than leaves. Ginkgo leaf extract contains trace amounts of ginkgotoxin. The amount of ginkgotoxin in ginkgo leaf and leaf extract seems unlikely to cause toxicity. However, there are anecdotal reports of seizure occurring after use of ginkgo leaf both in patients without a history of seizure disorder and in those with previously well-controlled epilepsy.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Both of these can lower your seizure threshold a bit, meaning they make it slightly easier for a seizure to happen. Ginkgo seeds contain a compound called ginkgotoxin that, in large amounts, can trigger convulsions. The good news is that the ginkgo leaf extract found in most supplements has only trace amounts, so the concern here is largely theoretical.
Still, there are a handful of anecdotal reports of seizures with ginkgo leaf, so it's worth a quick conversation. If you've ever had seizures, or you notice twitching, confusion, or unusual jerking movements, loop in your pharmacist or doctor before adding ginkgo. Please don't stop your medication on your own.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Some clinical research shows that a specific ginkgo leaf extract (Remembrance, Herbs Product LTD) 140 mg twice daily can induce CYP2C19 enzymes and potentially decrease levels of drugs metabolized by these enzymes. However, other clinical research shows that taking ginkgo 120 mg twice daily for 12 days has no effect on levels of drugs metabolized by CYP2C19.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combo product pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Amitriptyline is partly handled by the liver enzyme CYP2C19. The concern here is that some ginkgo extracts may speed that enzyme up (induce it), which could lower amitriptyline levels and make it work a little less well. Neither of these drugs is a prodrug, so faster clearance generally means weaker effect, not stronger.
Honestly, the evidence is shaky. One ginkgo product nudged this enzyme, while another study showed no effect at all, so this is mostly theoretical. I wouldn't lose sleep over it, but if your mood, sleep, or symptoms shift after starting ginkgo, mention it to your pharmacist or doctor before changing anything.

Laboratory research suggests that ginkgo leaf extract can mildly inhibit CYP1A2 enzymes. However, clinical research suggests ginkgo might not affect CYP1A2. Until more is known, use ginkgo cautiously in patients taking drugs metabolized by these enzymes.

This combination pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Part of how your body clears amitriptyline runs through a liver enzyme called CYP1A2. The thinking here is that ginkgo might slow that enzyme a little, which could let amitriptyline build up slightly. If that happened, you might notice more of its usual effects, like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.
Here's the honest part: this is mostly based on lab studies, and human research suggests ginkgo may not affect this enzyme much at all, so the real-world risk looks low. Ginkgo can also thin the blood, which is worth mentioning too. Before adding ginkgo, just check with your pharmacist or doctor.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro, a specific standardized extract of ginkgo leaf (EGb 761) inhibits CYP2C9 activity . The terpenoid (ginkgolides) and flavonoid (quercetin, kaempferol, etc.) constituents seem to be responsible for this effect. Most ginkgo extracts contain some amount of these constituents. Therefore, other ginkgo leaf extracts might also inhibit the CYP2C9 enzyme. However, clinical research suggests that ginkgo might not have a significant effect on CYP2C9 in humans. Ginkgo does not seem to significantly affect the pharmacokinetics of CYP2C9 substrates diclofenac or tolbutamide.

Here's the short version: this is a very theoretical concern. The idea is that ginkgo might slow down a liver enzyme called CYP2C9, which could let certain medicines build up a bit. With Triavil (amitriptyline plus perphenazine), higher levels could in theory mean more of the usual side effects, things like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.
That said, the evidence here is weak. It comes from lab studies, and human research actually suggests ginkgo doesn't meaningfully change this enzyme in real life. There's a separate, more practical point worth knowing: ginkgo can thin the blood a little, so before adding it, please run it by your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).
