Why this applies to Bazedoxifene Acetate, Conjugated Estrogens

Bazedoxifene Acetate, Conjugated Estrogens falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

The estrogen and bazedoxifene combo in Duavee is partly broken down by a liver enzyme called CYP3A4. The thinking here is that ginkgo might speed that enzyme up a little, which could lower the level of medication in your system. If that happened, the medicine might not work as well, so you could see hot flashes creep back or get less of the bone protection you are taking it for. This is not a prodrug, so it isn't a case of ginkgo making it stronger.

Honestly, the evidence here is weak and mixed. Most studies show ginkgo doesn't meaningfully change CYP3A4, and this concern is mostly theoretical. Still, since ginkgo can also thin the blood, it's worth a quick chat with your pharmacist or doctor before combining them.

P-GLYCOPROTEIN SUBSTRATES

Bazedoxifene Acetate, Conjugated Estrogens also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, taking ginkgo with P-glycoprotein substrates might increase the levels and adverse effects of these substrates.
A small clinical study in healthy volunteers shows that using ginkgo leaf extract 120 mg orally three times daily for 14 days can increase levels of the P-glycoprotein substrate, talinolol, by 36% in healthy male individuals. However, single doses of ginkgo do not have the same effect.

Here's the gist with Duavee (conjugated estrogens with bazedoxifene) and ginkgo. The bazedoxifene part is moved around your body by a transporter called P-glycoprotein. In a small study of healthy men, taking ginkgo for two weeks slowed that transporter enough to raise levels of a similar drug by about 36 percent. So in theory, regular ginkgo could nudge up how much Duavee stays in your system.

If that happened, you might notice more of its usual effects, things like breast tenderness, leg cramps, nausea, or fluid retention. This is mostly theoretical and a single dose of ginkgo didn't matter. Still, since Duavee affects clotting and ginkgo can thin the blood too, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
