Cenobamate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Golden Ragwort interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in golden ragwort.
Golden ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the thing to understand about this pairing: the worry isn't that Golden Ragwort changes how your Cenobamate works. It's the other way around. Cenobamate speeds up a liver enzyme called CYP3A4, and that same enzyme is what turns the pyrrolizidine alkaloids in Golden Ragwort into harmful, liver-damaging byproducts. So in theory, taking Cenobamate could ramp up that toxic conversion and raise your risk of liver injury from the herb.

I'll be honest, this is a theoretical concern based on lab and animal data, not strong human studies. But Golden Ragwort is already considered unsafe to swallow because of those liver-damaging compounds, so I'd steer clear of it entirely. Please talk with your doctor or pharmacist before adding any herbal product while you're on Cenobamate.
