Golden Ragwort and Zanubrutinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Why this applies to Zanubrutinib

Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Golden Ragwort interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in golden ragwort.
Golden ragwort contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the unusual twist with this pair: the concern isn't really about your cancer medicine getting stronger or weaker. It's about the herb itself. Golden Ragwort contains pyrrolizidine alkaloids, which your liver can turn into substances that harm liver cells and blood vessels. Zanubrutinib (Brukinsa) can speed up the same liver enzyme (CYP3A4) that creates those toxic byproducts, so in theory it could make Golden Ragwort more damaging to your liver, not less.

I'll be straight with you: this is a theoretical idea drawn from lab and animal work, not from real patient cases. But honestly, Golden Ragwort is already considered unsafe to swallow on its own. Please don't take it while on Zanubrutinib, and check with your oncology team or pharmacist before adding any herbal product.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Zanubrutinib page

“Grapefruit is the biggest supplement risk with zanubrutinib (Brukinsa), a targeted therapy for certain blood cancers, and this list deserves genuine care. Grapefruit blocks the enzyme (CYP3A4) that clears the drug, so levels can climb and side effects with them.”
