Acetaminophen, Phenylephrine, Chlorpheniramine falls under HEPATOTOXIC DRUGS — the group Gotu Kola interacts with.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the part of your cold medicine that matters most for this combo: the acetaminophen (the same ingredient as Tylenol). Acetaminophen is processed by your liver, and in high doses or with heavy alcohol use it can stress the liver. Gotu kola has a handful of case reports where people developed liver irritation while taking it, though we honestly can't say for sure the herb was the cause.
The concern is simply additive: two things that might bother the liver at the same time. This is theoretical and the evidence is weak, so don't panic. Just stay within the recommended acetaminophen dose, watch for warning signs like yellowing skin, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before pairing them, especially if you have liver concerns.
