Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's something worth knowing about pairing guggul with irinotecan. Irinotecan is actually a prodrug, meaning your body has to convert it into its active cancer-fighting form (called SN-38). Lab studies suggest guggul's active compounds, the guggulsterones, may speed up an enzyme called CYP3A4, which is one of the players involved in handling this drug. The worry is that revving up this enzyme could shift how much active medicine your body makes, potentially making your chemo less effective.

That said, this concern comes only from test-tube and animal research, not human studies, so it's theoretical at this point. Still, with a cancer treatment this important, you don't want to leave it to chance. Please check with your oncology team or pharmacist before adding guggul.
