Guggul and Oliceridine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Oliceridine

Oliceridine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Oliceridine (Olinvyk), the IV opioid given for short-term, severe pain. Your liver uses an enzyme called CYP3A4 to help break it down. Lab and animal studies hint that guggul may speed up that enzyme, which could clear the drug a little faster and make it less effective. In practice that might mean weaker pain relief than expected.

Oliceridine isn't a prodrug, so this isn't about boosting its strength. Honestly, the evidence here is thin (test-tube and animal data only), and since Oliceridine is given in a hospital where they monitor your pain and adjust the dose, this rarely becomes a real problem. Still, let your care team know about any guggul you take so they can keep your comfort on track.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Oliceridine page

“Oliceridine (Olinvyk) is an IV opioid used in hospitals for acute pain, and I treat its supplement list as one to respect, since eight of the 158 interactions on file rate major. Most of the list involves the liver enzymes that clear the drug: grapefruit can push oliceridine levels up and intensify side effects, while St.”
