Why this applies to Oliceridine

Oliceridine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Oliceridine (Olinvyk) is a powerful IV opioid given for short-term, severe pain in hospital settings. Your liver clears it partly through an enzyme called CYP2D6. The concern here is that DMAA, at least in lab studies, appears to block that same enzyme. If CYP2D6 is slowed down, oliceridine could clear more slowly and build up in your body, which may mean stronger opioid effects.

With an opioid, that could look like more drowsiness, confusion, very slow breathing, or dangerously low alertness. Honestly though, the bigger issue is DMAA itself, which is an illegal, unsafe stimulant that can spike your blood pressure and heart rate. Since oliceridine is hospital-given, this combo is unlikely in practice, but please mention any supplements to your care team.

Olinvyk (oliceridine) injection is approved for adults to manage acute pain that is severe enough to require an intravenous opioid analgesic. It is used only when alternative treatments are ineffective, not tolerated, or otherwise unable to provide adequate pain control.

Because of serious risks including addiction, misuse, overdose, and death, Olinvyk is reserved for situations where other pain management options are not sufficient. It is not intended for routine or chronic pain management.
