Higenamine and Acalabrutinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your body relies on a liver enzyme called CYP3A4 to clear acalabrutinib (Calquence). Higenamine has shown, in lab dish studies only, that it can partly block that same enzyme. If that happened in a real person, acalabrutinib could build up a bit higher than expected, which might mean more of its known side effects, things like bruising or bleeding more easily, headaches, low blood counts, or an irregular heartbeat.

Here's the honest part: this is theoretical. The blocking was weak (about 21%) and has never been confirmed in people. On top of that, higenamine itself can speed up your heart and raise blood pressure, which isn't ideal alongside a cancer drug. I'd skip it and check with your oncology team or pharmacist before adding any pre-workout or weight-loss supplement.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Acalabrutinib page

“For anyone taking acalabrutinib (Calquence) for chronic lymphocytic leukemia or lymphoma, the supplement list below deserves genuine scrutiny. Of 144 ingredients on file, nearly all act on CYP3A4, the liver enzyme that clears the drug, or on P-glycoprotein, a transporter that moves it, so they can push its level too high or leave too l…”

About Higenamine

What is Higenamine?

  • Weight loss and fat burning
  • Pre-workout energy and performance
  • Appetite suppression
  • Athletic stimulant
  • Traditional medicine remedies

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. It is banned in sports by the World Anti-Doping Agency (WADA), and product labels often list inaccurate amounts, so most people should approach it with caution and talk to a pharmacist or doctor first.

About Acalabrutinib

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
