Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's something important about Apadaz: the benzhydrocodone part is a prodrug. Your body uses an enzyme called CYP2D6 to turn it into hydrocodone, and then into hydromorphone, which is what actually relieves your pain. Higenamine has been shown in lab dishes to block that same CYP2D6 enzyme. If that blocking happened in real people, less of your medicine would get switched into its active painkilling form, so you might get weaker pain relief than expected.

The good news is this is only theoretical right now, based on test-tube research and never confirmed in humans, so I don't want to alarm you. Still, higenamine carries its own heart and blood pressure risks. Please check with your pharmacist or doctor before combining them.

In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the thing to know about Apadaz: the benzhydrocodone part is actually a prodrug, meaning your body has to convert it into hydrocodone before it can ease your pain. That activation, and a lot of the later breakdown, leans on a liver enzyme called CYP3A4. Higenamine has been shown in lab dish studies to slow that enzyme down by about 21 percent.

If that slowdown happened in real people, the active opioid could linger longer and pile up, which might mean more drowsiness, dizziness, constipation, nausea, or in a worst case slowed breathing. But honestly, this is only theoretical right now (it has never been shown in humans), so I don't want to alarm you. Higenamine carries its own heart and blood pressure risks too, so please run this combination past your pharmacist or doctor first.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
