Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the gist for Rexulti (brexpiprazole): your body clears it partly through a liver enzyme called CYP2D6. Higenamine has been shown in lab dish studies to slow that same enzyme down. If that played out in real life, brexpiprazole could build up a bit higher than expected, which might mean more of its usual side effects like drowsiness, restlessness, dizziness, weight gain, or feeling shaky.

I want to be honest about the evidence, though. This is theoretical and only seen in test-tube research, never confirmed in actual people. So it may not amount to much. Still, supplement labels often misstate how much higenamine is inside, and it can stir up the heart and blood pressure on its own. I'd run this combo by your pharmacist or doctor before mixing them.

In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the gist for Rexulti (brexpiprazole): your body relies on a liver enzyme called CYP3A4 to clear it out. In lab dish testing, higenamine slowed that enzyme down a bit (about 21%). If that happened in real people, brexpiprazole could build up a little higher than usual, which might mean more of its typical side effects like drowsiness, restlessness, dizziness, weight gain, or feeling lightheaded when standing.

That said, I want to keep this in perspective. This is only a theoretical concern from lab studies, and it has never actually been seen in humans. Brexpiprazole works as-is (it is not a prodrug needing activation), so the direction is simply 'possibly a bit stronger.' Higenamine also stresses the heart on its own, so before combining them, check with your pharmacist or doctor.

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
