Caffeine (OTC drug) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with combining higenamine and a caffeine pill like Vivarin or Overtime. The lab-based concern is that higenamine may slightly slow down an enzyme (CYP3A4) that helps clear certain drugs, which could let caffeine linger and build up a bit. But honestly, the bigger and more practical worry is that both are stimulants. Stacking them can stir up a racing or pounding heartbeat, jitteriness, anxiety, trouble sleeping, and higher blood pressure.
The enzyme part is only theoretical, based on a test-tube study (about 21% inhibition), and hasn't been shown in real people. Still, since both rev up your system, I'd run this combo by your pharmacist or doctor before taking them together.

Higenamine has stimulant effects due to agonist activity at beta2-adrenoreceptors. In cardiac muscle, higenamine appears to have a positive inotropic effect and increase heart rate. However, it does not appear to increase blood pressure.

Here's the thing about pairing higenamine with a caffeine pill like Vivarin: both are stimulants, and they push on your heart and nervous system in overlapping ways. Higenamine acts a bit like adrenaline, and caffeine winds you up too, so stacking them could add up. You might feel a racing or pounding heart, jitteriness, anxiety, trouble sleeping, or in some folks an irregular heartbeat.
I'll be honest with you, this concern is mostly theoretical, based on how these compounds work rather than strong human studies. Still, since higenamine products are often mislabeled and WADA-banned, I'd treat the combo with respect. If you're using both, please run it by your pharmacist or doctor first, especially if you have any heart or blood pressure history.
