Why this applies to Cevimeline

Cevimeline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to clear cevimeline. Higenamine appears to slow that enzyme down, at least in test-tube studies. If that happened in real life, cevimeline could build up a bit and its effects could feel stronger than usual. Since cevimeline already boosts saliva and other secretions, that might mean more sweating, watery eyes, runny nose, nausea, stomach cramping, or a slower heartbeat.

The big caveat is that this is only seen in lab research, not in people, so it's theoretical. Still, higenamine has its own heart and blood pressure concerns, so I'd run this combo by your pharmacist or doctor before mixing the two.

In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the gist for these two. Your body uses an enzyme called CYP3A4 to help clear cevimeline (Evoxac). In a lab dish, higenamine slowed that enzyme down a little (about 21 percent). If that happened in real life, cevimeline might hang around longer and its effects could be stronger than usual.

For cevimeline that could mean more of its typical cholinergic effects, like extra sweating, more saliva, a runny nose, nausea, or stomach upset. I want to be honest though: this is only a test-tube finding and has not been seen in actual people, so it's more of a theoretical caution than a proven problem. Higenamine itself can also stir up your heart rate and blood pressure, which is its own reason to be careful. Run it by your pharmacist or doctor before combining them.
