Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the gist: your body clears cobimetinib (Cotellic) mainly through an enzyme called CYP3A4. In lab dishes, higenamine slowed that enzyme down by about 21%. If that happened in a person, cobimetinib could build up a bit higher than expected, which in theory might mean stronger side effects like diarrhea, nausea, skin rash, vision changes, or a drop in blood pressure.

I want to be honest, though: this is only test-tube data and has not been seen in real people, so the actual risk is uncertain. That said, cobimetinib is a serious cancer medicine, and higenamine itself can stress the heart. Please don't start, stop, or change anything on your own. Run this combo past your oncologist or pharmacist first.

What Is Cobimetinib Used For?

Brand name: Cotellic

Cobimetinib is approved for two uses in adults. First, it is used in combination with another drug called vemurafenib to treat melanoma (a type of skin cancer) that cannot be surgically removed or has spread to other parts of the body — but only when the tumor carries a specific genetic change called a BRAF V600E or V600K mutation. A lab test must confirm this mutation before treatment begins.

Second, cobimetinib can be used on its own (as a single agent) to treat adults with histiocytic neoplasms — a rare group of cancers involving immune cells called histiocytes.
