Mexiletine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the situation with these two. Your body relies on a liver enzyme called CYP2D6 to clear mexiletine out of your system. In lab dish studies, higenamine appears to slow that same enzyme down. If that happened in people, mexiletine could build up to higher levels than expected, which may bring on more of its known side effects like nausea, dizziness, tremor, lightheadedness, or in a worst case, new or worsening heart rhythm problems.

I want to be honest about the evidence, though. This is only theoretical, seen in test tubes and not confirmed in actual patients. Still, mexiletine is a heart rhythm drug with a fairly narrow safety window, and higenamine itself can stress the heart. I'd skip combining them until you check with your pharmacist or cardiologist first.

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
