Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist with Erleada (apalutamide). Your body uses an enzyme called CYP3A4 to help break this medicine down. In lab studies, horny goat weed seems to slow that same enzyme. If that happened in people, apalutamide could clear a bit more slowly and build up, which in theory might mean stronger effects and more side effects like fatigue, joint pain, skin rash, high blood pressure, or, rarely, seizures.

I want to be honest about the evidence though: this comes only from test-tube research, not from real patients, so it's more of a theoretical concern than a proven one. Still, since apalutamide is a serious cancer drug, please run any supplement past your oncologist or pharmacist before adding it.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
