Why this applies to Chlorpheniramine, Pseudoephedrine
Chlorpheniramine, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo product pairs an antihistamine (chlorpheniramine) with a decongestant (pseudoephedrine). The concern here is that, in lab tests only, horny goat weed appears to slow down a liver enzyme called CYP3A4. Chlorpheniramine is partly handled by that enzyme, so in theory it could hang around a bit longer and you might feel more of its usual effects, like drowsiness, dry mouth, or dizziness.
Here's the honest part: this has only been seen in test tubes, never in real people, so I wouldn't lose sleep over it. Pseudoephedrine can already cause a racing heart or jitteriness, so just keep an eye out for that. Before adding horny goat weed, especially if you have heart issues, run it by your pharmacist or doctor first.
