Lonafarnib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Lonafarnib (Zokinvy) is the active drug itself, not a prodrug, and your body relies heavily on a liver enzyme called CYP3A4 to clear it out. In test-tube studies, horny goat weed appears to slow that same enzyme down. If that happened in people, less of the drug would be broken down, so it could build up and push its effects and side effects higher. With Lonafarnib that might mean more nausea, vomiting, diarrhea, poor appetite, or fatigue.

The important part: this is only laboratory data, and it has not been seen in humans, so we don't know if it truly matters. Still, Lonafarnib already comes with strict warnings about CYP3A4 interactions, so please check with your pharmacist or doctor before combining them.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
