Naproxen Sodium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with naproxen (the pain reliever in Midol Extended Relief). Your liver uses an enzyme called CYP1A2 to help break naproxen down. In a test-tube study, horny goat weed slowed that enzyme. If that happened in real life, naproxen could clear a bit more slowly and build up, which might nudge up its usual effects and side effects, things like stomach upset, heartburn, or a higher chance of stomach irritation.
The honest part: this comes only from lab work, not from people, so it's theoretical. I wouldn't lose sleep over it, but if you plan to use both regularly, run it by your pharmacist or doctor first, especially if you have stomach or heart concerns.

In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the gist with naproxen sodium (like Midol Extended Relief) and horny goat weed. Naproxen is an NSAID, and on its own it can irritate the stomach lining and make it a little harder for your blood to clot. That's why some people notice easy bruising, nosebleeds, or stomach upset while taking it.
In test tube and animal studies, horny goat weed seemed to slow platelets (the cells that help form clots) a bit too. Stack that on top of naproxen and the theoretical concern is a slightly higher chance of bleeding or bruising. I'll be honest though: this has not been shown in real people, so the evidence is weak. Still, if you take naproxen regularly, mention the herb to your pharmacist or doctor first, especially if you bleed easily or take other blood thinners.
