Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the basic idea with pentazocine (Talwin) and horny goat weed. Your liver uses an enzyme called CYP1A2 to help clear pentazocine out of your system. In lab tests, an extract from horny goat weed slowed that same enzyme down. If that happened in a real person, pentazocine could hang around a bit longer and build up, which might mean stronger effects and more side effects like drowsiness, dizziness, nausea, slowed breathing, or feeling foggy.

I want to be honest about the evidence, though. This is only seen in test-tube studies, not in actual people, so it's more theoretical than proven. Still, since pentazocine already carries sedation risk, it's worth a quick chat with your pharmacist or doctor before combining the two.
