Ponesimod falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the basic idea: your body uses a liver enzyme called CYP3A4 to help clear Ponesimod. In lab dish studies, horny goat weed seems to slow that enzyme down. If that happened in real life, Ponesimod could build up a bit more than usual, which might mean stronger effects and a higher chance of side effects like a slower heart rate, headache, dizziness, raised blood pressure, or changes in liver tests.

I want to be honest about the evidence, though. This has only been seen in test tubes, not in actual people, so it's theoretical at this point. Since Ponesimod is a serious MS medicine with real heart effects, I'd still run any horny goat weed use past your pharmacist or doctor first rather than guessing.

It is not approved for children, and it is only available as an oral tablet under the brand name Ponvory.
