Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist for Ranolazine (Ranexa): your body relies heavily on an enzyme called CYP3A4 to clear this medicine out. In test-tube studies, horny goat weed appears to slow that enzyme down. If that happened in real life, ranolazine could build up higher than expected, which may bring on more of its known side effects like dizziness, headache, nausea, constipation, or feeling lightheaded. At very high levels, ranolazine can also affect heart rhythm (a QT prolongation concern), so I don't take that lightly.

That said, this is only theoretical right now, seen in lab work and not confirmed in people. Still, since ranolazine has a narrow safe range, I'd run this combination past your pharmacist or doctor before mixing them.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
