Treprostinil (Orenitram) is a medicine that widens your blood vessels to ease the high pressure in your lungs. One of its built-in effects is lowering blood pressure, so it can already leave some folks feeling lightheaded, flushed, or dizzy when standing up. The concern here is that horny goat weed may also nudge blood pressure downward, so stacking the two could, in theory, drop your pressure more than expected.

I want to be honest about how thin this evidence is. The blood-pressure-lowering effect of horny goat weed has only been seen in lab and animal studies, never reported in actual people. So this is a possible, mostly theoretical caution. Still, since treprostinil affects your blood pressure directly, it's worth a quick chat with your pharmacist or doctor before adding this herb.

In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the thing to keep in mind with these two together. Treprostinil (Orenitram) is used for pulmonary arterial hypertension, and one of its known effects is that it slows down how well your platelets clump together. In other words, it already nudges you toward easier bleeding and bruising on its own.

Horny goat weed may do something similar. In lab dishes and animal studies, it appears to slow platelet activity too. So in theory, stacking it on top of treprostinil could add up to a bit more bleeding risk, think easy bruising, nosebleeds, or bleeding that's slow to stop. I want to be honest, though: this has only been seen in lab and animal research, not in people, so the real-world risk is uncertain. Still, it's worth a quick chat with your doctor or pharmacist before combining them.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
