Each serving has some grape seed extract, and in lab dish studies that extract appears to mildly slow down an enzyme called CYP2D6. Your body uses that same enzyme to clear dextromethorphan, so in theory the cough medicine could hang around a bit longer and build up slightly.
If that happened, you might notice more of dextromethorphan's usual effects, like drowsiness, dizziness, or feeling a little foggy. But I want to be honest: this is rated minor and unlikely, and it has never actually been reported in people. For most folks taking normal doses, this isn't something to lose sleep over.
