Here's the gist. The grape seed extract in these chews has shown, in lab dish studies only, a mild ability to slow down an enzyme called CYP2D6. Some of your methadone is handled by that enzyme, so in theory, blocking it could let methadone build up a little. Since methadone is an active drug (not a prodrug that needs switching on), higher levels could mean more of its usual effects, like extra drowsiness, slowed breathing, constipation, or dizziness.

That said, I don't want to scare you. This is rated minor and unlikely, methadone is actually cleared mostly by other enzymes, and there are no real reports of this happening in people. Still, because methadone is a powerful medicine where small changes matter, please run it by your pharmacist or prescriber before adding these chews.

Here is the gist for Methadone. The grape seed extract in these chews might slow down CYP3A4, one of the liver enzymes that helps break down methadone. In theory, if that enzyme is blocked, methadone could clear a little more slowly and build up a bit, which could nudge up its effects and side effects like drowsiness, slowed breathing, constipation, or feeling foggy.

That said, this is a very weak, lab-only concern. There are no real human reports of this actually happening, and the amount of grape seed extract in a couple of chews is small. Still, methadone is a medicine where even modest changes deserve respect, so I would simply mention these chews to your pharmacist or prescriber before adding them rather than worrying.

What Is Methadone Used For?

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
