Amitriptyline, Perphenazine falls under SEROTONERGIC DRUGS — the group Iboga interacts with.

Theoretically, taking iboga with serotonergic drugs might increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.

This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). The amitriptyline part raises serotonin activity in the brain, and Iboga's main alkaloid, ibogaine, also nudges serotonin upward. Stacked together, the worry is too much serotonin at once. In theory that could trigger serotonin syndrome, with symptoms like agitation, sweating, a racing heart, shivering, muscle twitching, or confusion.

Keep in mind this is a theoretical concern based on how these substances work, not proven cases. Honestly, the bigger issue is that ibogaine itself is illegal in the U.S. and can cause dangerous heart rhythm problems, and both of these medicines can affect heart rhythm too. Please don't change anything on your own. Talk with your doctor or pharmacist first.

Here's the concern with this pairing. Triavil combines amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and both of these can stretch out the heart's electrical rhythm, something we call QT prolongation. Iboga, through its ibogaine content, has been linked in several real-world case reports to this same effect. Stacking them together could add up, and in rare cases that raises the risk of a dangerous, fast heart rhythm that may cause fainting, palpitations, or worse.

Honestly, Iboga isn't something I'd recommend in the first place, since it carries serious heart and safety risks on its own and is illegal in the U.S. The heart-rhythm evidence here is based on case reports, not strong trials, but with a medicine like Triavil it's worth taking seriously. Please loop in your doctor or pharmacist before mixing anything like this.

Here's the idea behind this one. Triavil combines amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and both lean on anticholinergic activity, meaning they block a brain chemical called acetylcholine. In lab dish studies, iboga seems to raise acetylcholine levels, which could theoretically push back against that blocking action and make the medicine work a little less than expected. Interestingly, the main compound ibogaine didn't show this effect, so the concern is shaky at best.

To be honest, this is very weak, test-tube-only evidence, and the bigger worry by far is that iboga itself carries serious, even fatal, heart rhythm risks. Please don't combine these without talking to your doctor or pharmacist first.

Amitriptyline, Perphenazine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to know: your medication, the amitriptyline plus perphenazine combo (Triavil), can act as a CYP2D6 blocker. Iboga's active compound, ibogaine, is cleared by that same CYP2D6 pathway. So if you took them together, your medication could slow down how your body breaks ibogaine down, letting it build up to roughly double the usual levels. The worry isn't your antidepressant getting stronger here, it's the ibogaine piling up. Ibogaine on its own can trigger dangerous heart rhythm changes and has been linked to deaths.

Honestly, I'd steer clear of iboga entirely. It's illegal in the U.S. and carries serious cardiac risks even by itself. Please loop in your doctor or pharmacist before mixing anything new with Triavil.
