Why this applies to Clomipramine

Clomipramine falls under ANTICHOLINERGIC DRUGS — the group Iboga interacts with.

Theoretically, iboga might decrease the effectiveness of anticholinergic drugs.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

Here's the idea on file: in lab dishes (not in people), Iboga seemed to raise acetylcholine levels by blocking an enzyme that normally breaks it down. Since clomipramine (Anafranil) has its own anticholinergic activity (the drying-out effects like dry mouth, constipation, blurred vision, and trouble urinating), the theory is that Iboga could push against those effects a bit. Honestly though, this is very weak, theoretical evidence, and the main Iboga compound, ibogaine, did not show this effect.

The bigger concern isn't this gentle theory at all. Both clomipramine and ibogaine can affect heart rhythm and serotonin, so combining them could be genuinely dangerous. Iboga is also illegal in the U.S. Please don't pair these without talking to your doctor or pharmacist first.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS

Clomipramine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: in this pairing, it's the Iboga that gets affected, not your Clomipramine. Clomipramine is one of the drugs that slows down the CYP2D6 enzyme, and ibogaine (Iboga's active compound) depends on that same enzyme to be cleared from the body. So with Clomipramine on board, ibogaine and its breakdown product can pile up. One small study found levels roughly doubled. That matters because Iboga can cause dangerous heart rhythm changes, and higher levels make that risk worse.

On top of that, both Clomipramine and Iboga raise serotonin, which can stack into a serious reaction. I'd strongly urge you not to combine these. Iboga isn't legal in the U.S. anyway, but please talk with your doctor or pharmacist before changing anything.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
