Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down crizotinib (Xalkori) and keep its blood levels in a safe range. Piperine, the active compound in Indian long pepper, can slow that same enzyme down a bit. In theory, that means crizotinib could clear more slowly and build up, which might intensify its effects and side effects.

With crizotinib that could look like more nausea, vomiting, diarrhea, swelling, vision changes, or fatigue, plus a possible impact on heart rhythm or liver enzymes. I want to be honest though: this is based only on lab (test tube) data, not real patient studies, so it's a theoretical concern, not a proven one. A pinch in cooking is unlikely to matter, but a concentrated supplement is worth a conversation. Please check with your oncologist or pharmacist before using it regularly.

Here's the gist for Xalkori (crizotinib): your body uses a kind of cellular pump called P-glycoprotein to help move this drug out of cells and clear it. Piperine, the main compound in Indian long pepper, may slow that pump down. In theory, that could let a bit more crizotinib build up in your blood, which can mean stronger side effects like nausea, diarrhea, swelling, vision changes, fatigue, or shifts in liver tests.

I want to be honest about the evidence here. This concern comes only from lab (test-tube) studies, not from real patients, so it's theoretical. Everyday seasoning amounts are unlikely to be a problem, but concentrated supplement doses are less predictable. Since crizotinib is a serious cancer medicine, please check with your oncology pharmacist or doctor before using long pepper supplements.

About Crizotinib

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) treats adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for ALK or ROS1 gene changes, as confirmed by an FDA-approved test.

Xalkori is also used for pediatric patients (age 1 and older) and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL), and for adult and pediatric patients (age 1 and older) with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
