Here's the gist with Tivdak (tisotumab vedotin-tftv). This is a powerful IV cancer medicine your oncology team gives you, and part of it (the MMAE payload it carries) is broken down by a liver enzyme called CYP3A4. In lab dishes, piperine from Indian long pepper appears to slow that same enzyme down. In theory, if it slows the breakdown, more of the drug could linger in your body, which might mean stronger side effects like nausea, fatigue, hair loss, nerve tingling in the hands and feet, or eye irritation.

I want to be honest though: this is theoretical and based only on test-tube data, not real patients. Still, since this is a serious chemo-type drug, please mention any long pepper supplements to your oncologist or pharmacist before using them. Occasional culinary spice is far less of a concern than concentrated capsules.
